Radial Tunnel Syndrome (Radial Nerve Entrapment)
What is the condition?
Radial tunnel syndrome is pain on the outer side of the elbow and upper forearm caused by irritation or compression of a branch of the radial nerve, most often the posterior interosseous nerve, as it passes through muscles just below the elbow.
It can feel similar to tennis elbow, but the pain is typically deeper and slightly further down the forearm, within the muscle bulk rather than directly over the bony prominence. This diagnosis can be challenging because there is no single definitive test and nerve conduction studies are often normal.
What are the symptoms?
Symptoms are usually a deep aching pain on the outer elbow and upper forearm that is worse with repetitive forearm use, gripping, and activities that involve turning the forearm or extending the wrist and fingers.
Some patients describe a fatigue-type pain that builds with activity. Tingling is not always present because the affected nerve branch may mainly carry pain fibres rather than sensation. True weakness is uncommon in pure radial tunnel syndrome and should prompt a careful assessment for other diagnoses.
How is it diagnosed?
Diagnosis relies on careful clinical assessment and exclusion of other causes of lateral elbow pain, particularly tennis elbow. Tenderness is often maximal in the forearm muscles along the nerve course rather than at the lateral epicondyle.
Imaging is used selectively to exclude other problems such as arthritis, tendon tears, or a mass. Nerve studies may be helpful to exclude other neuropathies, but a normal study does not exclude radial tunnel syndrome.
Non-surgical treatment
Treatment focuses on reducing provocative activities, improving ergonomics, and a structured physiotherapy programme to reduce overload and restore mechanics.
Anti-inflammatory medication may help in the short term if appropriate. In selected cases, an injection may be used diagnostically or therapeutically.
Surgical treatment
Surgical decompression is considered when symptoms remain significant despite adequate non-surgical treatment and the clinical picture is consistent with nerve entrapment. Surgery aims to release potential compression sites along the nerve in the radial tunnel.
Recovery and rehabilitation
Recovery is gradual and typically focuses on early motion, scar management, and progressive return to use. Improvement may take weeks to months, particularly if symptoms were present for a long time.
Expected outcomes
Many patients improve with appropriate conservative management. When surgery is required, outcomes are best when the diagnosis is clear and other causes of lateral elbow pain have been excluded.
Risks and complications
Risks include infection, stiffness, persistent symptoms, scar tenderness, and nerve irritation. Because lateral elbow pain can have more than one contributor, ongoing symptoms can occur even after technically successful decompression.
When should I seek urgent review?
Seek urgent medical review if you develop increasing redness, heat, rapidly worsening swelling, fever, increasing pain that does not settle with rest, or new weakness or numbness in the hand. If you have had a recent injury and the elbow looks deformed, feels unstable, or you cannot move it, this also requires urgent assessment.
Surgery, Risks and Recovery
Surgery is considered when symptoms persist despite appropriate non-operative treatment, when imaging or nerve studies confirm structural pathology, or when weakness, locking, or mechanical symptoms are present.
Operations are usually performed as day procedures under regional or general anaesthetic. Techniques depend on the underlying condition and may involve tendon debridement or repair, arthroscopic bone spur removal, loose body excision, fracture fixation, ligament reconstruction, nerve decompression, or joint replacement.
General risks include infection, bleeding, stiffness, scar sensitivity, nerve irritation, persistent symptoms, recurrence, and the need for further surgery. Condition-specific risks are discussed prior to surgery.
Recovery varies according to the procedure performed. Gentle motion is usually encouraged early. Wounds typically heal within two weeks. Light activities resume over two to four weeks for minor procedures, while larger reconstructions or replacements may require several months of rehabilitation.
Patients often notice improvement within six to twelve weeks, although full recovery may continue for up to a year depending on the pathology treated.
